On
July 30, 2026, at approximately 3:00 p.m., Jake Jakubowski was last
seen leaving the Mountain of Mercy facility in the Mattole area.
Staff reported that he left the property on foot, and his current
whereabouts are unknown.
Jake
is described as a White male adult, approximately 6 feet 3 inches to
6 feet 4 inches tall, weighing 150 to 160 pounds, with blonde hair
and blue eyes.
He has a large port-wine birthmark on his right arm.
He has no other known distinguishing scars, marks, or tattoos.
Jake
has a reported medical condition that may affect his perception,
judgment, or ability to safely care for himself, making him an
at-risk missing person.
Out of respect for his privacy, no additional
medical information is being released.

Sheriff’s deputies are currently canvassing the area in an attempt
to locate Mr.
Jakubowski.
